You are provided coverage by this insurance from the day you start working. Allgeier Air pays the full cost.
Coverage and Benefits as a result of injury from accidents while on the job include:
Reasonable medical costs
Weekly benefits after (7) days of being off work due to the accident.
Permanant disability benefits
Death Benefits
In the event of any accident or injury to you on the job, you must IMMEDIATELY report the injury to your supervisor.
The reporting of Injuried and accidents is to go up the chain of command to the Leadership Team and/or HR.
You will be given first aid treatment for your injuries. You may also be instructed where to go to have your injuries cared for by a medical professional.
As necessary you will be driven, or in the case of a severe accident an ambulance may be called.
Any Accident or Injury will automatically require a drug screening. You will be driven to the location for the drug screening to be perfommed. This typically will be the same location where your injuries will be attended to.
You may be asked if the injury was while you were on the job. Answer the question appropriately. The hospital/immediate care center will then call our office for approval for Workers Compensation treatment.
Do not give your insurance card to the hospital/immediate care center.
The Company offers group medical insurance for both single and married employees. The Company pays one half (½) of the employees single coverage.
All Health Insurance Premiums beyond One Half (1/2) the individual premium, including family coverage if desired, are paid by the employee and are deducted from the Team Member's Payroll.
Insurance for becomes effective beginning the first full Month after 60 days of Employment.
It is important to fill out the Insurance Participation Form at the time of your onboarding with the Company.
After 90 days of employment you will be offered dental insurance. It is strictly on a voluntary basis - the company does not offer any money toward this insurance.
If you have any questions about coverage or claims procedures, you may consult your policy or check with the HR. They can answer any questions that you might have or obtain answers from the insurance Company.
The HR Manager may direct you to a representative of the Insurance company to answer any questions.
If you are a regular full time employee and you resign, are terminated from the employment of Rick Allgeier & Associates Inc, or if your work hours are reduced, you and your eligible dependents will have the right to continue to participate under COBRA for up to eighteen (18) months at your, or your dependents' expense.
If you are determined to be disabled under the Social Security Act at the time of your termination or are working reduced hours, you may be entitled to continuation of coverage for up to twenty-nine (29) months.
Your eligible dependents may also extend coverage at their expense for up to thirty-six (36) months under our group health insurance plan in the event of your death, divorce, legal separation, entitlement to Medicare benefits, or when a child ceases to be eligible for coverage as a dependent under the terms of the plan.
The eighteen (18) month continuation coverage period provided in the event of your termination, or reduction of working hours may be extended to thirty-six (36) months for your spouse and dependent children, if within that eighteen (18) month period you die, become divorced, legally separated, or a child ceases to have dependent status.
In addition, if you become eligible for Medicare during the eighteen (18) month period, your spouse and dependent children will be entitled to extend their continuation period to thirty-six (36) months, starting on the day that you become eligible for Medicare. If this election for continuation coverage is made, you have the option to convert this policy to an individual policy with our insurance carriers at the end of the continuation period.
Continuation coverage may end, however, if any of the following events occur:
Failure to make timely payments of all premiums.
Assumption of coverage under another group health plan which does not exclude or limit coverage provided to you on account of pre-existing medical condition.
The termination by Allgeier Air of its group health plans.
If you become entitled to Medicare.
You will no longer be eligible for continued coverage, but, as noted earlier in the section, your spouse or dependent children may be entitled to extend their continuation coverage.
Our plan administrator will contact you about these options at the time termination occurs or your work hours are reduced. The plan administrator will also contact your qualified beneficiaries in the event of your death or entitlement to Medicare benefits. However, in the event you become divorced or legally separated or one of your dependents ceases to be eligible for coverage under our group health plans, you and/or your dependents are responsible for contacting the Company and the plan administrator to discuss continuation/conversion plan rights. You and your qualified beneficiaries are also responsible for contacting the plan administrator within sixty (60) days of qualifying for Social Security disability benefits. For further details regarding continuing or converting your group health benefits, please contact the Office Manager.
After 90 days of employment you will have a $30,000.00 (thirty thousand) life insurance policy provided by Allgeier Air. No employee contribution required.
Because the Company recognizes the importance of vacation time in providing an opportunity for rest, recreation and personal activities, Allgeier Air grants Paid Vacation Time to its full-time regular employees.
Vacation Time, also known as paid time off (PTO) accrues each week. The Accrual rate is as follows:
The amount of PTO that Allgeier Air provides represents a substantial amount more than most companies allow in the first or second year.
First year: Accrual rate equals 1 Full Hour Per Week. Total of 52 hours per year.
After (2) years of service: Accrual rate doubles to 2 Full Hours Per Week. Total of 104 hours per year
There are currently no additional PTO benefits beyond the Weekly PTO Accrual.
Allgeier Air provides an accrual of vacation time beginning your first full week of employment.
Allgeier Air grants Paid Vacation Time to its Full-Time Regular Team Members.
Vacation may be taken as hours accrue at any time during the year but only after the first six months of employment.
Time off work is important for recharging your batteries, therefore Team Members must take Vacation Time (PTO)
Team Members are allowed to carryover a portion of their PTO time. The maximum amount of Vacation time allowed to be carried forward to the next calendar year is equal to their weekly accrual rate x 52 weeks.
For example: In your 1st or 2nd year As a Team Member, if you have(or will have by Dec 31st) accrued 60 hours of Vacation Time (PTO). You must use 8 hours of PTO in order to meet the "Maximum Accrual Carryover"
If you do not use the amount of PTO that is over the allowance for carryover you will lose that amount beginning January 1st. There will be no Payment in lieu of using the Vacation Time.
The amount of carryover doubles at the end of the 3rd year onward.

There will be no payment in lieu of taking the PTO time. In the event that a Team Member leaves the Company that Team Member forfeits all PTO Time accrued up to that point.
Vacation/PTO time taken for more than three days at one time, must be scheduled with and approved by your direct supervisor, and HR manager.
You are required to fill out a "Time Off Request" Form that can be provided by your Direct Supervisor.
Along with the team member signature, the time off approval also requires the signature of both the Direct Supervisor, and HR Manager.
Your Direct Supervisor will attain the approval with the HR dept. and/or Upper Management as soon as possible, and at least 30 days in advance.
Approval time required for PTO taken for less than 3 days must be scheduled as soon as possible and at least 7 days in advance.
PTO will not be approved for the following:
Any period beyond 5 consecutive working days.
Call in due to illness, or other circumstance that requires you to miss work
Time off due to any Worker Compensation Claim that may be active
Time off due to short hours for any given pay period
The Company provides paid time off to all full-time regular employees after ninety days of service on the following scheduled holidays:
New Years Day
Memorial Day
Independence Day
NOT Columbus Day
Labor Day
Thanksgiving Day
Christmas Day
In order to receive holiday pay, you are required to work the regularly scheduled hours(full day) in the workday preceding and the workday following the holiday. In accordance with Company policy, an approved vacation day or any other excused and paid day off is considered a day worked for purposes of holiday pay eligibility.
Employees will furnish their own hand tools. A list of tools required for your success is included elsewhere in this Manual. Any tools not listed will be decided upon as needed.
The Foreman does not furnish tools for others working for him. It is up to each individual to have his own tools.
Borrowing tools from other Team Members may be necessary on occasion. However if this occurs more than necessary the Team Member who is borroing the tools should make arrangements to provide that particular tool for themselves.
Certain Tools are Provide by the Company. These include but are not limited to Large equipment such as
Vacuum Pumps, and Torches.
Any damage or loss of Company or employee tools will be replaced by the person responsible for the damage or loss.
